Submit a guardianship referral

The Upper Valley Community Board of Guardians accepts referrals for adults who may need guardianship and who appear to have no appropriate person available to serve. Referrals are reviewed individually and are subject to eligibility, capacity, legal requirements, conflicts, available funding, and board approval.

Before you begin

  • The individual is believed to be an adult
  • The individual lives in Jefferson, Madison, Teton, Fremont, or Lemhi County
  • There are concerns about the individual's ability to make or communicate important decisions
  • Less restrictive alternatives have been considered
  • No appropriate family member, friend, or other person appears available to serve
  • The referral is not being submitted only because services, housing, or discharge planning have become difficult

This is not an emergency service

How your referral is handled

Referrals are sent over an encrypted connection and stored in a private record that only authorized board reviewers can open after signing in. Every view and decision is recorded. Documents you attach are uploaded straight into encrypted private storage and can only be opened by an authorized reviewer through a short-lived secure link.
